Transition to adult services experienced by young people with cerebral palsy

Infographic entitled Transition to adult services experienced by young people with cerebral palsy: A cross-sectional study.  Long description : The infographic provides questionnaire results that assessed if young people with cerebral palsy experience and health professionals provide practices that may improve transition from child to adult health services.  The percentage of young people reporting each practice was: appropriate parent involvement 90%; health self-efficacy 37%; named worker 36%; self-management support 36%, transition information 24%; meet adult team 16%; and life skills training 16%. The percentage of health professionals reporting each practice was: appropriate parent involvement 66%; health self-efficacy 73%; named worker 35%; self-management support 73%; transition information 69%; meet adult team 30%; life skills training 36%; and senior manager 20%.  The results indicate many young people do not receive supports that may improve the experience of transition
